Hearing Loss: Early Signs and When to See a Doctor

Hearing loss is often called the "invisible disability" because it develops gradually and silently. By the time most people realize they have a hearing problem, significant damage may have already occurred. As an ENT specialist, I see patients every week who have been struggling with hearing difficulties for years before seeking help. Early detection makes a tremendous difference in outcomes.

How Common is Hearing Loss?

Hearing loss is more common than most people realize. Over 63 million people in India suffer from significant auditory impairment. It affects people of all ages — from newborns to the elderly — and can have profound effects on communication, education, employment, and mental health.

Types of Hearing Loss

Conductive Hearing Loss

Caused by problems in the outer or middle ear that prevent sound from reaching the inner ear. Often treatable with medication or surgery. Common causes include ear wax, fluid in the middle ear, perforated eardrum, or otosclerosis.

Sensorineural Hearing Loss

Caused by damage to the inner ear (cochlea) or auditory nerve. Usually permanent but manageable with hearing aids or cochlear implants. Causes include aging, noise exposure, viral infections, and certain medications.

Mixed Hearing Loss

A combination of both conductive and sensorineural hearing loss. Requires comprehensive evaluation and a combined treatment approach.

Early Warning Signs

These are the early signs of hearing loss that you should never ignore:

  • Frequently asking people to repeat themselves
  • Difficulty understanding speech in noisy environments
  • Turning up the TV or radio volume higher than others prefer
  • Missing parts of conversations, especially on the phone
  • Difficulty hearing high-pitched sounds (birds, doorbells)
  • Tinnitus — ringing, buzzing, or hissing sounds in the ears
  • Feeling that people are mumbling or speaking unclearly
  • Withdrawing from social situations due to difficulty hearing
  • Difficulty hearing consonants (s, f, th, sh sounds)
  • Needing to lip-read to understand conversations

Common Causes of Hearing Loss

  • Age-related hearing loss (Presbycusis) — natural decline with aging
  • Noise-induced hearing loss — prolonged exposure to loud sounds
  • Ear infections — chronic otitis media can damage hearing
  • Ear wax buildup — temporary but significant hearing reduction
  • Ototoxic medications — certain antibiotics, chemotherapy drugs
  • Viral infections — mumps, measles, meningitis
  • Head trauma — injury to the ear or skull
  • Genetic factors — hereditary hearing conditions

Hearing Tests We Perform

Pure Tone Audiometry

Measures hearing sensitivity across different frequencies. The gold standard hearing test.

Speech Audiometry

Tests ability to hear and understand speech at different volumes.

Tympanometry

Evaluates middle ear function and eardrum mobility.

OAE Testing

Otoacoustic emissions test — assesses inner ear (cochlear) function.

BERA/ABR

Brainstem Evoked Response Audiometry — tests the auditory nerve pathway.

Hearing Aid Trial

Fitting and evaluation of appropriate hearing aids when needed.

Treatment Options

  • Ear wax removal — simple procedure with immediate improvement
  • Medications — for infections, sudden hearing loss, autoimmune conditions
  • Hearing aids — for sensorineural hearing loss (many modern, discreet options)
  • Cochlear implants — for severe to profound hearing loss
  • Surgery — for conductive hearing loss (tympanoplasty, stapedectomy)
  • Auditory rehabilitation — speech therapy and listening training

Protecting Your Hearing

  • Use ear protection in noisy environments (concerts, construction sites)
  • Keep headphone volume below 60% of maximum
  • Take listening breaks — follow the 60/60 rule
  • Treat ear infections promptly
  • Get regular hearing check-ups, especially after age 50
  • Avoid inserting objects into the ear canal
  • Manage cardiovascular health — poor circulation affects hearing
