Understanding Sinusitis: Causes, Symptoms & Treatment

Sinusitis is one of the most common conditions I treat in my ENT practice. It affects over 30 million people in India every year, yet many patients suffer for months before seeking proper treatment. In this article, I'll explain everything you need to know about sinusitis — from what it is to how it's effectively treated.

What is Sinusitis?

Sinusitis is inflammation of the sinuses — the air-filled cavities in your skull that surround your nose. When these cavities become inflamed or infected, they fill with fluid, causing the characteristic pressure, pain, and congestion that sinusitis patients experience.

There are four pairs of sinuses: frontal (forehead), maxillary (cheekbones), ethmoid (between the eyes), and sphenoid (behind the nose). Any or all of these can be affected by sinusitis.

Types of Sinusitis

Common Causes

• Viral infections (common cold) — most common cause of acute sinusitis
• Bacterial infections — often follow viral sinusitis
• Allergic rhinitis — chronic inflammation from allergens
• Nasal polyps — growths that block sinus drainage
• Deviated nasal septum — structural blockage
• Dental infections — upper tooth infections can spread to sinuses
• Environmental irritants — smoke, pollution, dry air

Symptoms to Watch For

The classic symptoms of sinusitis include:

• Facial pain or pressure (especially around cheeks, forehead, and eyes)
• Nasal congestion and blocked nose
• Thick yellow or green nasal discharge
• Reduced sense of smell and taste
• Headache, especially in the morning
• Tooth pain (upper teeth)
• Fatigue and general malaise
• Post-nasal drip causing sore throat or cough

Diagnosis

As an ENT specialist, I diagnose sinusitis through a combination of:

• Detailed medical history and symptom assessment
• Nasal endoscopy — direct visualization of the nasal passages
• CT scan of sinuses — gold standard for chronic sinusitis
• Allergy testing — to identify underlying allergic triggers
• Nasal swab culture — to identify bacterial infections

Treatment Options

Treatment depends on the type and severity of sinusitis:

Medical Treatment

• Nasal saline irrigation — clears mucus and reduces inflammation
• Nasal corticosteroid sprays — reduces swelling and inflammation
• Antibiotics — for confirmed bacterial infections (10–14 days)
• Antihistamines — for allergy-related sinusitis
• Decongestants — for short-term relief of congestion
• Mucolytics — to thin and clear thick mucus

Surgical Treatment (FESS)

Functional Endoscopic Sinus Surgery (FESS) is recommended for chronic sinusitis that doesn't respond to medical treatment. It's a minimally invasive procedure that opens blocked sinus passages and removes polyps or diseased tissue. Most patients experience significant improvement in symptoms after FESS.

Prevention Tips

• Stay hydrated — drink 8–10 glasses of water daily
• Use a humidifier in dry environments
• Practice nasal irrigation with saline solution
• Avoid known allergens and irritants
• Treat allergies promptly
• Wash hands frequently to prevent viral infections
• Avoid smoking and secondhand smoke
